For a short, recent absence — a state-licensed provider reviews your request and, if it's medically supportable, signs a note you can hand to work or school.
Notes cover up to 3 days each, for mild, self-limiting conditions, and you'll usually have it within about an hour.
Start the intake, and a state-licensed provider completes your FMLA (Family and Medical Leave Act) certification or leave of absence paperwork, field by field, to the form's requirements. The provider certifies the medical facts; your employer or leave administrator decides eligibility and approval.
If your employer or leave administrator sent a specific form (Amazon DLS, Sedgwick, or another employer's packet), upload it and the provider completes that one.
A state-licensed provider can write a workplace accommodation letter, or complete a specific accommodation form your employer sent (like a Leave as an Accommodation request), describing the accommodation you're requesting and its medical basis.

The provider's real license and credentials are on it. Acceptance is your employer, school, or leave administrator's call, not Certicare's, so we don't promise an outcome we can't control.
The limits are part of how we keep it that way. You must be 18 or older. Doctor's notes cover up to 3 days each, with at most 6 days of notes in any rolling 30-day period, for mild, self-limiting conditions. Every document is dated and signed the day it's completed — nothing backdated or post-dated. If a request isn't medically supportable, a provider declines it and you're refunded in full.
