What we can complete

One intake covers a range of documents. Doctor's notes for work and school. Return-to-work notes. FMLA (Family and Medical Leave Act) certification and leave of absence (LOA) paperwork. A Request for Information (RFI) — the follow-up an administrator sends asking for more detail. Workplace accommodation letters or forms.

If your employer, school, or a leave administrator sent you a specific form, a state-licensed provider completes that form, field by field. If a request isn't something a provider can medically support, we tell you and refund you.

Why this holds up

A state-licensed provider personally reviews your request before completing anything, and your employer or school can verify the document directly. The provider's real license and credentials are on it. Acceptance is your employer, school, or leave administrator's call, not Certicare's, so we don't promise an outcome we can't control.

The limits are part of how we keep it that way. Doctor's notes cover up to 3 days each, with at most 6 days of notes in any rolling 30-day period, for mild, self-limiting conditions. Every document is dated and signed the day it's completed — nothing backdated or post-dated. If a request isn't medically supportable, a provider declines it and you're refunded in full.
