A short online intake, usually with no appointment. A state-licensed provider reviews your answers and, if it's medically supportable, completes the exact form your employer or school requires, then emails it to you ready to submit. A provider may follow up for more detail before deciding.
Start My IntakeDoctor's note $29 · Leave and accommodation paperwork $49
For a short, recent absence, a state-licensed provider reviews your request and, if it's medically supportable, signs a note you can hand to work or school.
$29 · usually within about an hour · up to 3 days per note
A state-licensed provider completes your FMLA certification or leave of absence paperwork field by field, including a specific form your employer or leave administrator sent.
$49 flat per case · usually within 24 hours after we have everything needed
A state-licensed provider writes an accommodation letter, or completes a specific form your employer sent, describing the accommodation and its medical basis.

One intake covers a range of documents. Doctor's notes for work and school. Return-to-work notes. FMLA (Family and Medical Leave Act) certification and leave of absence (LOA) paperwork. A Request for Information (RFI) — the follow-up an administrator sends asking for more detail. Workplace accommodation letters or forms.
If your employer, school, or a leave administrator sent you a specific form, a state-licensed provider completes that form, field by field. If a request isn't something a provider can medically support, we tell you and refund you.
A state-licensed provider personally reviews your request before completing anything, and your employer or school can verify the document directly. The provider's real license and credentials are on it. Acceptance is your employer, school, or leave administrator's call, not Certicare's, so we don't promise an outcome we can't control.
The limits are part of how we keep it that way. Doctor's notes cover up to 3 days each, with at most 6 days of notes in any rolling 30-day period, for mild, self-limiting conditions. Every document is dated and signed the day it's completed — nothing backdated or post-dated. If a request isn't medically supportable, a provider declines it and you're refunded in full.
No hidden add-ons — the price you see is the price you pay.
For medical leave and accommodation paperwork, the $49 covers your full case — the forms it needs, plus any verification, clarification, or corrections your employer or leave administrator requests, at no extra charge.
